Our Director of Nursing and Nursing Supervisor have over 50 years of nursing experience between them and they have trained our field staff to be the eyes and ears of your physician. You can rest assured; they will monitor your condition in a way that is second only to being in your physician’s office.

COMMUNICATION: You are not in the hospital but in many ways, you may require the kind of coordinated care a hospital offers. The only way we can provide that in the home is with a very open and frequent dialogue among the field staff and with you. They in turn can communicate your needs and their impressions with your case manager and physician. This will make it possible for us to provide you with great coordinated care. We are committed to successful communication which will make it possible for your physician and PHHI to provide you with hospital level care in your own home.
